Abstract | The social determinants of health (SDOH), including socioeconomic status, education, neighborhood environment, and access to healthcare, significantly influence individual and population health outcomes. These determinants create disparities that often lead to unequal access to care and health inequities. Addressing SDOH is particularly critical in tertiary care settings, where patients frequently present with complex medical and social needs (Marmot et al., 2008). Tackling these multifactorial issues requires a multidisciplinary approach that integrates social care with clinical care to achieve improved health outcomes and equity. |
